Indexing Application

IMPORTANT UPDATE

INDEXING PORTAL IS LIVE!

You can now index using our online system to complete your indexing application.

There is a $750 charge for an individual indexing and $1,500 charge for a family. 

Visit Indexing Information page for instructions on how to submit an indexing application for an individual or family.  Include primary indexing applicant's name and contact email on payment form. 

 

Once payment has been processed, a confirmation email will be sent and you will receive an invoice receipt for you to keep for your records. You will be notified when your payment has been received and when your indexing application has been fully processed.
